Awesome holiday!

After had routine activities and made so bored, yah it’s time for having new experience and adventure! I got my annual leave : 11 Sept until 18 Sept 2018.. Yeayyyy!!!! My big boss signed it.. Where will i go?? Fortunately that my best friend, dude will come to indonesia!! Okay, just being confident with myelf cuz my english isn’t good.. And i dunno can i have another chance for meeting him in real? So, i decided for having trip with him.

11 Sept 2018, my dad accompanied me to Sepinggan International Airport. He gave me some advices. Seriously i love my dad so much cuz he supported everything. I checked in ticket and i don’t need baggage cuz i bought backpack with 40l capacity. In the departure gate, i am so happy and i wanna shout ‘i am freeeeeeee’ but i changed my mind cuz it’s public area. I don’t want make myself as attention. I took a seat for waiting my flight. 30 minutes later i had conversation with Mbak Mei. The topic started from ‘do u wanna go to surabaya too?’ which city will you go? And became deep conversation. We shared our opinion about social environment (mostly about religion). It was 9.15 am, time for flying. The woman which wear pink hijab gave us smile. We introduced each other. Her name is Mbak Puji. She wanna go to Perak for visiting her fiancee. Wow.. So sweet.. We just knew few minutes ago, but we were like know each other for a long long time. That’s silly things. In the plane, we promised to hang out after had trip from surabaya. I got my seat at 16F. My favorite seat near the window. And a boy sat beside me. I am trying to make conversation with him. His name is Wahyu. He wanna move to samarinda. He went to blitar for taking his education certificate. His dad is army. We talked a lot of topics. He like for trekking the mountain. And i am so lucky for meeting him. He has dream to be pilot. Wow.. Cool guy! This is the 1st time i didn’t sleep in my flight haha.

9.45 am, finally arrived at Juanda International Airport. Wahyu gave his hand for taking my backpack. We went out from plane. I was shaking his hand as our farewell. ‘Let’s your dream come true, wahyu’

Welcome to surabaya!!!!!! My first destination was Malang. Cuz dude already at there (exactly kaffi’s boarding house). The weather so hot similar like Balikpapan. Taking 4 hours for going to Malang. That was terrible traffic. Huuffttt.. In my way, dude and kaffi always sent me messages.. What time will you arrive here? I thought that dude can’t wait for visiting some interesting places. At 3 pm finally i arrived at kaffi’s BH. It was the 1st time i met my best friend in real. Yah.. When i am with him, i just realize that i am too short! Oh yah.. Not only dude, but i met kaffi, panjul, alex, kitana, ruri, and yoga. I met them before in my company cuz two months ago they were trainee. The first place which we visited was restaurant. Oohhhh.. I am so hungry. We ate so much. Finishing our lunch, we prepared for visiting my best friend -indah- which got married one month ago. Seriously, i have never met indah before. And it was 1st time i met her in real. The things that i believe, friendship is built by good communication. I was happy when i met her. Kaffi and the ganks accompanied me went to there. She has good personality, so kind with us. She gave us one box apple juice and one box corn crispy. We were happy cuz we have snack for eating in Bukit Bintang. Bukit Bintang is romantic place for watching stars and city’s light. We acted so crazy in that night. I was happy cuz dude made good adaptation with us. Snack wasn’t enough for us. We went for eating Bakso in front of Museum Angkut. Hhmmmm.. Yummy Bakso. It was 11.30 pm, i was so sleepy and we decided to go home. Hey Malang, goodnight. Pls being nice to me.

Next day, 12 Sept 2018. We walked one km for waking them up (kaffi’s and the ganks). Because of they had any classes, so they didn’t go with me and dude. After making breakfast for them, i and dude decided to go to Jatim Park II. Why i choose Jatim Park II? Cuz it’s zoo and i have never visited zoo. We used GrabCar for going to there. The driver was woman. But i forgot her name hehe. She asked who is dude? Where i met him? She was happy to have conversation with dude. We made deep conversation until she shared her problem to me. Because it’s secret, i won’t tell it in this blog. Sorry! Too private!

Arriving at Jatim Park II Yeayy.. I got discount 20% dan dude got 30% off. He wore t-shirt with orang utan picture. It matched with our destination. ZOO! I was so happy cuz there are so many apes species. I like apes so much! Btw, i met new friend. Her name is Novy. She is dude’s friend. She just graduated from UMM with Biology Majority. We shared many topics. First, i was so afraid that we can’t making friend. But it’s working.. Finally we became friend! I was excited with orang utan. But other side, i was so sad ‘they didn’t stay in their habitat”. We spent 5 hours for walking around JP II. Seriously, i didn’t feel hungry. But dude did it.

After that, we went to outdoor store for buying jacket and shirt. Cuz we will go to Bromo for watching sunrise. Yeayyyyyy!! Bromo was one of my bucket list destination! Dude got army jacket and t-shirt. Time showed 10.00 pm, we must go to home for taking a rest. I tried my best for closing my eyes but i can’t. 00.05 am we arrived at KFC Kawi which meeting place for BROMO Trip. I paid 300k and dude 550k. For information, there is difference between local and tourist for enterance fee. I just knew and it was my 1st time having trip with foreigner. Not only me, there were two newlywed couples. And for once again, they asked me how can i meet with dude? *is it strange for having friend from another country? For me it’s normal.

It was long road with not good condition. I tried to make myself sleep. But i can’t. I suggested my brain. I imagined that i saw a huge savanah in front of me. I listened a song from asteriska with the title song ‘savanah’. Finally i am okay with that condition. 03.15 am we arrived at Penanjakan I. I was enthuasism for watching sunrise. I ran fast, i can’t wait anymore!!!! Arriving at there, hhuuffftt.. There were so many people! Mostly foreigner. I can’t see anything. They were so tall. I really wanna take an epic pict. I tried to be brave to communicate with them. I asked for giving me a little space. They were smiling and laughing when i said that i am too short! Btw do u know? Dude fell so cold and wore his lovely blanket there. The temperature was 5 deg celcius. His body was trembling! He was mumble but i tried to make him calm down. Just enjoy this trip dude. You can face this cold.

I shared many topics with him : The reason i am using ‘v.nillasky’ as my instagram name, why i like so much with mountain, romantic defined me.

After watching sunrise, we decide to eat instant noodles. I thought that dude was hungry. He dislike instant noodles. We ate instant noodles under the sun shine. He said ‘it’s romantic’

Mas wira told that we must return to jeep at 6 am. But we didn’t care. All people in jeep were waiting us. Haha.. I am sorry everyone!

Hey Bromo, good morning! 13 Sept 2018. Next destination = Bromo crater. Seriously, Bromo has beautiful scenery. The thing that made me so regret, i can’t take a picture. Mas wira drove so fast! I have one task = trekking Bromo Crater! We were so enthuasism. I was so tired but i must have done it. Just push ur limit hening, u can! (supporting myself)! Finally i did it with my own foot! Without any help (riding a horse). But many thanks for dude. Thank you for supporting me. Thank you for giving me a hand. The next destination : Lautan Pasir Berbisik and Bukit Teletubbies. I thought cuz of tired, we were not enthuasiasm with this place!!

It was 11.30 am, mas wira drop us off to Malang Train Station. Probowangi’s train departure was 03.45 pm. It was too early. So we decided to have lunch. I ate soto and dude ate Fish Baked. The reason why he always eat Fish in indonesia = In berlin, it has high price aka expensive! Huuaaahh we were so full. We sat in the park opposite with Malang Train Station. I met Pak Nur. He is Angkot driver. We shared many topics. He was happy with dude. *i think every people likes dude*. He is old optimistic man with young spirit. Because dude can’t speak indonesia so he doesn’t know our conversation. He was bored. And he took a nap in the park!

3.00 pm, we decided to enter the station. Our train will be stop at track 3. I was so sleepy cuz i haven’t sleep yet. I told to dude that i must sleep in the train. Do u know what thing that makes me happy? I saw beautiful sunset from my train. I can’t see it in balikpapan. I dunno i am so warm everytime i see sunset. Is it so cliche?

The day became dark. It means night already came. I tried to sleep but i can’t. I saw dude feel not okay. I thought that the seat wasn’t comfortable for him. He was so terrible. I moved to another seat to give him chance for sleeping.

Finally, he slept! Dude, have a nice dream! – i know, you are so tired –

00.20 am we arrived at Rogojampi Train Station, Mas Husain dan Mas Sukmo pick us up. I was so sleepy but i had small conversation with them. Mas sukmo asked me to sleep. Okay it’s time for sleep. It’s too late. I slept in dyah’s room. Good night banyuwangi. I will make memory here for the 2nd time.

5.00 am, i woke up cuz dyah came to the room. She wanna go to school. What?? Isn’t it too early dek for going to school? She said that she had extra classes!! *seriously, in my life i never go to school at 5.00 am! I had small topic with this smart girl. I like her so much. She is smart, has brown skin and look so sweety. She never use make up. Dyah is creative person. She made a lot of crafts in her room. She wrote her dreams in sticky note and hanging it to wall. She has a dream for study abroad (germany). She likes so much albert einstein. *dyah, i wish all the best for you, and your dreams will be come true*. Then, dyah went to school and i continued my sleep.

9.00 am, i woke up from my good sleep. I dunno what should i do? So i swept all the room and washed dirty plates. I fed snack for kid (mas sukmo’s pet). I waited dude to wake up. I don’t want to disturb him. One hour later, he woke up! We were so hungry so we went to Warung Bu Poer. Then, dude went to mosque for friday praying.

12.00 am, he came back to home, we talked about many topics. At the moment, i was so shy because i didn’t have huge knowledge. He gave me knowledge about middle east, politics, etc. *i promised, i will read more books after this trip*. We talked so much. Finally, we decided to go to rice field around mas sukmo’s house. We walked through village. Everyone looked at me. Maybe cuz i was with dude. This is my favorite part. I proud to be indonesian. Everytime i smiled to them, they gave us the best smile too. *i rarely found this atmosphere in balikpapan*. I and dude like artist. Dude gathered to them. He said “hello or apa kabar”. It made everyone was happy. Do u know dude, which part that i like so much from you? When you gave a hand for grandma. You helped to bring her bag. *i am so gratefull cuz i have a kind friend like you”. I met the old man too. He harvested the lemongrass. And he gave me much lemongrass but dude asked me for return it. Yeahh i took only one. *some people think that it’s nothing, but for me it’s priceless. That lemongrass formed his appreciate. I am trying to respect everything including little things*

At night, i picked yana and kaffi up at Rogojampi Train Station. They joined with us for trekking to Ijen Crater. That was my first time to meet yana. Btw, yana is kaffi’s Girlfriend. Yana is polite, gentle and friendly girl. I like her so much. After picked them up, we slept for a while cuz we need extra energy. At 00.15 am we started our trip. 00.30 am we prepared our equipment like flashlight, jacket, enterance ticket, etc. Enterance fee for local was 7.5k and for tourist was 100k. Dude pretended as local. But he failed. Cuz he is too tall. I think maybe 20% of indonesian which have height more than 170 cm. 1.00 am we started our trekking. Ahhh.. For you know, Ijen Mountain has height 2.800 mdpl. There are so many people “local and tourist”. I saw most of them had high spirit. Cuz i never go trekking, it made me tired easily. My spirit never die! I must see blue fire even once in my life. Lesson learned that i got from this trekking = never give up for chasing everything! It wasn’t easy, but i can! Dude, Mas Husain and Kaffi helped me. *i love them so much*. 3.30 am we arrived at Ijen Crater. I cried! Finally i was there! Mas husain asked me “wanna see blue fire?”. I said yes ofc. We should go down. Aahh?? I was afraid cuz it’s rocky crater. He convinced me that i will be alright. Time for going down. I tried to be brave. It was so dark with rocky contour. 4.00 am i saw blue fire directly. With my own eyes! For once more, i cried. It’s so awesome beautiful. I am so lucky cuz i saw it!

Gagal Menikah!!!

Dari judulnya mengerikan yak. But it happens to me! Di usia saya yang sekarang ini, sudah sewajarnya orang tua saya mulai khawatir dikarenakan anaknya tidak kunjung menikah! Jadi melalui tetangga, saya dikenalkan saya dengan seorang pria. Saya pun tidak menolak, dan mencoba untuk menjalaninya. Dia menghubungi saya duluan. Saya anggap dia punya itikad baik kepada saya. Kami pun chatting ala kadarnya. Dan tidak intens. Di akhir bulan Maret 2018, saya melakukan perjalanan ke banyuwangi-bali. Tetapi, handphone saya hilang. Dia bilang selama perjalanan dia menghubungi saya tetapi tidak ada respon (yah namanya jugak hape ilang). Pertengahan bulan april pun saya kembali ke balikpapan. Tetangga saya pun menyampaikan pesan darinya. Okay, berlanjutlah hubungan ini. Seminggu setelah perjalanan, dia pun datang kerumah untuk mengenal lebih lanjut saya dan keluarga saya. Saya ini tipe orang yang berusaha tidak negative thinking kepada orang lain. Intinya yang ada di pikiran saya saat itu adalah dia punya itikad baik. Kenapa saya menolak hal-hal yang baik? Dan berjalanlah hubungan ini. Dua bulan menjalaninya seperti pasangan normal biasa, dia memberikan perhatian yang rutin kepada saya. Dan bulan berikutnya mulai lah banyak problematika. Dia jarang berkomunikasi dengan saya. Saya sih cukup menyadari bahwa dia sibuk dengan pekerjaannya. Ya paling keki nya ketika WA ga diread cuman online mulu. Wahh.. Sabar deh nghadapi orang yang sikapnya seperti ini. Setiap orang kan sikapnya beda-beda yak. Saya masih berpikiran positif bahwa tipekal orang yang seperti itulah dia. Meskipun banyak gak cocoknya, saya pun tetap bertahan dengan hubungan ini. Sampai akhirnya agustus lalu, dia dan orangtuanya datang untuk melamar saya. Bukannya dengan lamaran, hubungan kami tambah langgeng malah sebaliknya. Dan disinilah mulai lah terombang ambing hubungan kami. Tak terselamatkan.

Sebenarnya dari awal saya sudah merasa bahwa kami tidak cocok. Dan itupun dilihat dari sikapnya :

1. Sering membatalkan janji, meskipun itu janji untuk bertemu kedua orang tua saya.
2. Tidak ada usaha untuk mengenal lebih lanjut orang tua saya. Siapa yang ga kesel setiap ngjemput dan nganter gak pernah masuk kerumah?
3. Childish. Kalo ngambek itu bisanya block Whatsapp.

Dan saya pribadi yang gak doyan drama. Saya tipe yang langsung cari jalan keluar kalo ada masalah. Di sisi lain pun dia tidak bisa menerima saya dan dunia saya. Travelling is part of me. Dan setiap saya bercerita tentang dunia travelling, selalu saja mukanya ndak enak banget!

Btw gagalnya pernikahan ini itu cuman gegara perbedaan kami dalam rencana penyelenggaraan pernikahan. Dari awalnya dia memberikan uang sejumlah Rp. xxxxx dengan acara resepsi sederhana di rumah. Kami pun setuju dengan rencana itu. Lalu seminggu kemudian dia berubah pikiran. Dia minta di gedung. Dengan uang segitu yang menurut saya hanya cukup untuk biaya makan dan bayar jasa masak saja. Saya utarakan lah hal itu ke dia. Yang saya dapatkan jawaban dari dia adalah ‘kamu menyepelekan uang segitu hanya untuk makan’. Saya realistis dengan keadaan sebenarnya. Wahh.. Ini orang gimana ya?? Plin plan banget. Dari situ mungkin dia gak interest lagi dengan saya. Tetapi saya yakin setiap peristiwa yang terjadi itu ada kebaikan di dalamnya.

Tak ada niat mau menjelekan dirinya. Saya yakin setiap orang itu baik. Hanya kami yang tidak cocok jika disatukan.

Tapi dengan adanya ini banyak hikmah yang saya dapatkan :

1. Belajar bersabar
2. Lebih ikhlas dengan apa yang sudah Alloh tentukan
3. Orangtua menjadi lebih pengertian

Untuk next, saya tidak akan membohongi perasaan saya lagi. Saya akan juga belajar bagaimana menjadi dewasa. Dan ternyata umur itu bukan patokan kedewasaan seseorang. Saya juga berjanji dengan adanya peristiwa ini tidak akan membuat saya sedih dan terpuruk. Malah sebaliknya. Saya harus hidup lebih bahagia.

Pada akhirnya saya masih bisa melanjutkan hobby saya. Dan saya bahagia 😀

Jika Menikah, tetap kerja atau fokus menjadi Ibu Rumah Tangga?

Judul tersebut dilontarkan boss saya di kantor pagi ini. Hmmm.. *langsung garuk kepala hehe. Finally jawaban saya : tergantung dengan kondisi, apakah suami memberikan ijinnya kelak kepada saya atau tidak? Setiap pilihan pasti memiliki resiko ya dan kita tidak bisa pungkiri hal tersebut.

Jika tetap bekerja, yah asiknya ngumpul-ngumpul sama teman kantor, ketawa-ketiwi, punya banyak koneksi tapi disisi kurang meng-enak-annya kurang waktu bersama anak (jika sudah memiliki anak). Jika bekerja, setidaknya perempuan ikut andil dalam proteksi terhadap sesuatu yang tidak diinginkan spt suami diphk atau terjadi insiden (semoga gak ya).

Jika Fokus menjadi Ibu Rumah Tangga, punya banyak waktu bersama anak. Belajar dan bermain bersama. Mungkin bisa tiap hari ng-trip bareng anak yah melihat sunset atau bermain di pantai. Tetapi suatu saat akan merindukan suasana ramenya kantor hehe.

Untuk saya pribadi, saya mulai bekerja dari lulus SMK, 2 minggu setelah saya dinyatakan lulus alhamdulillah dapat kerjaan. Jika ditanya, selama ini saya betah banget ya kerja disana. Ya problematika pekerja pasti ada tetapi kita harus bisa mencari solusi yang tepat dan cepat. Problem tsb jangan dibawa berlarut-larut dan mengganggu kualitas kinerja kita. Jika bosan dan lelah, maka ambil cuti (refresh your mind).

Jadi bagaimana hening tetap kerja atau fokus menjadi Ibu Rumah Tangga?

Arti ‘perjalanan’ bagi hening

Malam ini pada ribut di group sebelah mengenai akomodasi untuk cross country go to Ijen 2018. Ada salah seorang yang plin plan semula minta akomodasi di homestay lalu berubah jadi VIP Resort. Dia minta rincian fasilitas untuk ngbandingin mana yang lebih bagus? Pada kasus saya, yang semula saya nginap dengan ABCD, kemudian berubah menjadi WXYZ, That’s okay! It’s not problem for me girls!

Bagi saya melakukan perjalanan adalah sebuah pembelajaran. Pembelajaran menjadi pribadi yang open minded. Perjalanan bukan hanya jalan-jalan atau selfie untuk dipublikasikan ke media sosial.

Dengan siapa kamu sekamar? Dari sini kamu belajar bagaimana beradaptasi dengan seseorang yang belum kamu kenal dan tentunya menjadikan kamu pribadi yang open minded (Setiap manusia punya cara pikir yang beda2, so yuk mari hargai!)

Akomodasi yang diluar dari ekspetasi? Salah satu tujuan melakukan perjalanan adalah keluar dari rutinitas yang membosankan aka zona nyaman. Sesekali ng-rasain kesederhanaan lah.. Karena bahagia itu terkadang berasal dari hal-hal kecil nan sederhana.

Selama melakukan perjalanan, banyak yang bisa saya ubah dari diri saya :

1. Jadi pribadi yang humble, meskipun ga kenal tetap tersenyum. Makanya cukup terkenal sekarang di fungsi lain.
2. Putus asa? Gak ah..
3. Open minded. Si A bicarakan tentang ini ga boleh itu ga boleh. Yowiiss orangnya seperti itu cukup dengarkan!

Yah masih segitu sih perubahannya meskipun gak signifikan yang penting ada kemajuan kecil lah..

Dow’s Fire & Explosion Index

The title reminds me about my thesis. And one of my lecturer disagree and attacks me with much questions about my thesis. But i really don’t care with him. I was interested in that title when i was in 5th semester. He told me it is like trash. Oh who do u think u are? Forget it.. And finally i got A for the final exam, I am so happy.

Back to the topic now. I wanna share my knowledge about one of method that used by some engineer to identification fire and explosion hazard.  This metode usually used in oil and gas company or petrochemical company where the chemical always used for much  activities.

Determine Process Unit

The important thing that you should ensure for choose the process unit  :

Process Unit will result loss and catastrophic impact if fire and explosion occur. And another requirement is containing more than 600 gallons of flammable, combustible or reactive material which connected with other process units (not single design).

Reference from AICHE, Some factors that we must ensure are :

1. Material Factor
2. Quantity of Hazardous Material
3. Catastrophic impact and fund density
4. Operation condition (exactly : pressure and temperature)
5. Past experience / accident (especially about fire and explosion)

Determine Material Factor

Material Factor give illustration about energy which is released because fire and explosion occur.  how we can get value of material factor? it’s so easy, just multiplying value of reactivity (Nr) and value of fire (Nf).  see this table :

Example : Gasoline

Gasoline has 3 for fire and 0 for reactivity (see NFPA 704), Gasoline is flammable liquid which will result of fire and explosion. So, Material factor of gasoline is 16 (see the table once again)

Determine General Process Hazard Factor (F1)

General Process Hazard is important for determine how big the fire will result? General Process Hazard Factor consist of six components.

1. Exothermic Chemical Reaction
2. Endothermic Process
3. Material Handling and Transfer
4. Enclosed or Indoor Process Units
5. Access
6. Drainage Spill Control

U must give penalty for all of  components based on ur operation condition and design of equipment.  Next step is adding all of penalty.  And the result is score of General Process Hazard Factor.

Determine Special Process Hazard Factor (F2) 

Special Process Hazard Factor is specific process operation which will expand incident probability and contribute with cause of fire and explosion occur. Special Process Hazard Factor consist of 12 components.

1. Toxic Material
2. Sub Atmospheric Pressure
3. Operation in or near Flammable Range
4. Dust Explosion
5. Relief Pressure
6. Low Temperature
7. Quantity of Flammable / Unstable Material
8. Corrosion and Erosion
9. Leaking – Joint and Packing
10. Use in Fired Equipment
11. Hot Oil Heat Exchange System
12. Rotating Equipment

U must give penalty for all of  components based on ur operation condition and design of equipment.  Next step is adding all of penalty.  And the result is score of Special  Process Hazard Factor.

Determine Process Unit Hazard Factor (F3)

Process Unit Hazard Factor is combination from General and Special Process Hazard. This factor usually contribute to fire and explosion occur. Determining this factor with multiplying General and Special Process Hazard.

Formula :     F3      =      F1  x  F2

If the calculation result more than 8, u must use this score. Because the maximum score for F3 is 8. 

Determine Fire and Explosion Index (F&EI)

Fire and Explosion Index illustrates level of risk in the process unit.

Formula     :   F&EI    =   MF   x   F3

where, MF = Material Factor,  F3 = Process Unit Hazard Factor

From that table, we can determine level of fire risk in our process unit. Therefore, we can know what we should do to control that hazard.

In this case, u can use engineering controls, administrative controls and protective personal equipment as hazard control.

Engineering Controls:

• Layout  (Separate the process unit with others which will result catastrophic impact)
• Spacing (Giving space between process unit and public area), ref. refining safety code for minimum recommended separation distance
• Safety Device and Alert System
• Fire Detector

Adminitrative Controls:

• Work permit
• Job Safety Analysis
• Training
• MSDS

Protective Personal Equipment :

• Fire Retardant Clothing
• Self Contain Breathing Apparatus

Cara Menghitung Frequency & Severity Rate (ANSI)

Sebuah perusahaan tambang batu bara memiliki data sebagai berikut :

1. Jumlah Jam Kerja = 272.796 hari
2. Fatality = 1 orang –> 6000 hari (berdasarkan tabel ANSI)
3. LTI Non Fatality = 2 Orang
4. LTI = 185 hari
5. Restricted Work Injury = 1 orang
6. Jumlah hari bekerja ringan = 10 hari
7. Medical Treatment Injury = 2 orang
8. First Aid = 10 hari

Dari data tsb, tentukan :

• LTIFR (Lost Time Injury Frequency Rate)
• LTISR (Lost Time Injuey Severity Rate)
• TRIFR (Total Recordable Injury Frequency Rate)
• TRISR (Total Recordable Injury Severity Rate)

Keselamatan dan Kesehatan Kerja

Philosophy Keselamatan dan Kesehatan Kerja ialah Upaya untuk menjamin keutuhan dan kesempurnaan tenaga kerja dan manusia pada umumnya, hasil karya dan budayanya menuju masyarakat yang adil dan sejahtera.

Tujuan Keselamatan (Safety) :

1. Mengendalikan kerugian dari kecelakaan (control of accident loss)
2. Kemampuan untuk mengidentifikasi dan menghilangkan (mengontrol) resiko yang tidak bisa diterima (the ability to identify and eliminate unacceptable risks).

Incident ialah kejadian yang berhubungan dengan pekerjaan yang menimbulkan atau hampir menimbulkan cidera, sakit, atau kematian (OHSAS 18001 – 2007). Kejadian yang tidak direncanakan atau tidak diinginkan yang menimbulkan atau hampir menimbulkan cidera/sakit, kerusakan asset dan pencemaran lingkungan.

Definisi Kecelakaan Kerja :

1. Menurut UU No. 1 Tahun 1970, kecelakaan kerja adalah suatu kejadian yang tidak diduga semula dan tidak dikehendaki, yang mengacaukan proses yang telah diatur dari suatu aktivitas dan dapat menimbulkan kerugian baik korban manusia atau harta benda.
2. Frank E. Bird dan George L. Germain mendefinisikan kecelakaan sebagai suatu kejadian yang tidak diinginkan yang menimbulkan kerugian pada manusia, kerusakan properti, ataupun kerugian proses kerja, sebagai akibat dari kontrak dengan substansi atau sumber energi yang melebihi batas kemampuan tubuh, alat, atau struktur.
3. Heinrich (1980) mendefinisikan kecelakaan sebagai suatu kejadian yang tidak terencana dan tidak terkontrol yang merupakan aksi atau reaksi dari suatu objek, substansi, manusia atau radiasi yang memungkinkan menyebabkan injury.
4. ILO (1989), kecelakaan merupakan kejadian yang tidak terencana dan terkontrol, yang disebabkan oleh manusia, situasi/faktor lingkungan atau kombinasi dari faktor-faktor tsb yang mengganggu proses kerja, yang dapat (ataupun tidak) menimbulkan injury, kesakitan, kematian, kerusakan properti, atau kejadian yang tidak diinginkan.

Bahaya atau ”Hazard” adalah suatu situasi atau keadaan yang dapat menyebabkan terjadinya kejadian yang tidak diharapkan yang dapat menghasilkan akibat kerugian berupa cedera manusia, kerusakan asset atau peralatan, atau kerusakan / penurunan daya lingkungan hidup.

Resiko / Risk adalah Kombinasi antara “kecendrungan” terjadinya keadaan atau paparan yg membahayakan dan “keparahan” dari cidera atau sakit yg disebabkan oleh kejadian atau paparan tersebut. (OHSAS 18001 – 2007).
